Survey for people diagnosed with dementia

Alzheimer’s Australia is currently conducting a survey about the experiences of people living with dementia. This survey will be used to help inform Alzheimer’s Australia work to create dementia-friendly communities across Australia. We are asking people living with a diagnosis of dementia to complete the survey, which can be found online here http://www.surveymonkey.com/s/dementiafriendly

The responses will provide Alzheimer’s Australia with information about the most important aspects of a dementia-friendly community for those living with dementia and what could be improved to help people to live well in the community. There survey has 10 questions and all responses to the survey will be treated anonymously. Hard copies of the survey are also available by contacting Kylie Watkins on 02 6278 8934 or kylie.watkins@alzheimers.org.au
